Composition
Each uncoated tablet contains:
Thyroxine Sodium IP — 50 mcg
Excipients: q.s.
Therapeutic Category
Thyroid Hormone Replacement / Hormonal Therapy.
Dosage & Administration
Route: Oral
Usual adult starting dose: 25–50 mcg once daily, increased gradually by 25–50 mcg every 2–4 weeks as per clinical response and TSH levels.
Typical maintenance dose: 50–200 mcg daily.
Administration:
Take once daily on an empty stomach, at least 30–60 minutes before breakfast.
Swallow with water; do not chew or crush.
Consistency of brand and timing is important for stable thyroid control.
Special dosing considerations:
Elderly or cardiac disease: Start at 12.5–25 mcg/day; titrate slowly.
Pregnancy: Dose may require increase; monitor TSH each trimester.
Warnings & Precautions
Monitor thyroid function tests (TSH, T₄) regularly to ensure adequate dosing.
Use cautiously in cardiac patients, as excess hormone may precipitate angina or arrhythmia.
Diabetic patients may require adjustment of antidiabetic medication.
Adrenal insufficiency must be corrected before starting therapy.
Avoid abrupt discontinuation.
Do not interchange brands without physician supervision due to possible differences in bioavailability.
Adverse Effects
Usually occur only with overdosage or excessive dosage, mimicking hyperthyroidism:
Common: Palpitations, tremor, anxiety, insomnia, sweating, heat intolerance, weight loss.
Less common: Tachycardia, arrhythmias, muscle weakness, diarrhoea, menstrual disturbances.
Rare: Allergic rash, angina, headache.
Use in Special Populations
Pregnancy: Safe and essential; dose adjustment often required.
Lactation: Compatible; minimal passage into breast milk.
Pediatrics: Individualized dosing based on age and weight.
Elderly: Start with lower doses; titrate cautiously.
Overdosage
Symptoms: Tachycardia, tremor, anxiety, hyperthermia, vomiting, cardiac arrhythmias, and hypermetabolism.
Treatment:
Discontinue levothyroxine temporarily.
Provide symptomatic treatment (β-blockers, supportive care).
In severe cases, cholestyramine may be used to enhance hormone excretion.
Storage
Store below 25°C, in a cool, dry place, protected from light and moisture.
Keep tightly closed and out of reach of children.
