Composition

Each uncoated tablet contains:

Thyroxine Sodium IP — 75 mcg
Excipients: q.s.

Therapeutic Category

Thyroid Hormone Replacement / Hormonal Therapy.

Dosage & Administration

Route: Oral administration.

Usual adult dose:
25–200 mcg once daily, depending on age, body weight, and disease severity.

Typical maintenance dose:
75 mcg once daily (as per tablet strength).

Administration:

Take once daily on an empty stomach, preferably in the morning, at least 30–60 minutes before breakfast.

Swallow whole with water; do not chew.

Titration:
Start at a low dose (e.g., 25 mcg/day) and increase gradually every 2–4 weeks based on clinical response and serum TSH levels.

Warnings & Precautions

Monitor thyroid function (TSH, T₄) regularly, especially after dose adjustments.

Cardiovascular disease: Use cautiously in patients with angina, arrhythmia, or hypertension.

Diabetes mellitus: May alter insulin or antidiabetic drug requirements.

Adrenal insufficiency: Correct before starting levothyroxine.

Elderly: Start at lower doses due to reduced cardiac reserve.

Do not switch between brands without medical supervision (bioavailability differences may exist).

 Adverse Effects

Usually related to overdosage or excessive dosage, mimicking hyperthyroidism:

Common: Palpitations, tremor, anxiety, sweating, insomnia, weight loss.

Less common: Tachycardia, arrhythmia, diarrhoea, muscle weakness, menstrual irregularities.

Rare: Allergic skin reactions.

Use in Special Populations

Pregnancy: Safe; requirements often increase — monitor and adjust dose accordingly.

Lactation: Compatible; minimal excretion in breast milk.

Pediatrics: Dose individualized based on age and weight.

Geriatrics: Start at lower doses due to cardiac sensitivity.

 Overdosage

Symptoms: Symptoms of hyperthyroidism — tachycardia, agitation, tremor, fever, vomiting, cardiac arrhythmias.
Treatment:

Symptomatic and supportive care (β-blockers for tachycardia).

Temporary discontinuation of therapy.

In severe cases, consider cholestyramine to enhance thyroxine elimination.

 Storage

Store below 25°C, in a cool, dry place, protected from light and moisture.
Keep tightly closed and out of reach of children.
