Warnings & precautions

Lactic acidosis (metformin): rare but serious — risk increases with renal impairment, dehydration, sepsis, hepatic impairment, excessive alcohol intake or conditions that predispose to hypoxia. Monitor renal function before and during treatment. European Medicines Agency (EMA)

Use caution when combining with insulin or insulin secretagogues (e.g., sulfonylureas) due to increased risk of hypoglycaemia; dose adjustments of the other agent may be required. MIMS

Adverse effects (most commonly reported)

Metformin-related: gastrointestinal (nausea, diarrhoea, abdominal discomfort), taste disturbance; rare lactic acidosis.

Vildagliptin-related: headache, dizziness, hepatobiliary enzyme increases, rarely pancreatitis or hypersensitivity reactions.

Combined therapy adverse events are often similar to the components; hypoglycaemia risk increases when used with insulin or sulfonylureas. 1mg+1

Use in pregnancy & lactation

Use only if clearly needed and after assessing potential benefits vs risks; limited data — consult specialist guidance and product information. Breastfeeding mothers should seek medical advice before use.

Overdose

Symptoms: hypoglycaemia (if combined with insulin/SU), GI upset, possible lactic acidosis with metformin overdose. Manage supportively; in severe metformin-associated lactic acidosis, consider urgent medical care and possible haemodialysis.

Store as per manufacturer label (usually at room temperature, protected from moisture and light). Keep out of reach of children.
